What is a W-9 Form?

Only U.S. residents who won a prize in a project that was organized by jovoto Inc. - our office in New York -, have to fill out this form.

Basically, W-9 is a form where you swear and provide your tax id number (either for the business or personal SSN (Social Security Number) and say what type of entity, (a person/sole proprietor, corporation, etc) you are.
This becomes the information that jovoto Inc. uses to provide a 1099 (please see this Wikipedia article for further informtion) or other tax report/form as (or if) required. And as penalties for any side to report wrong information can be severe, if you provide them with wrong information, for example say the wrong SSN, you will be held responsible.

Please go to ”Your Profile”, “Edit your Profile” and then ”Payment Information”. There you can download the W-9 form and also upload it again. Jovoto Inc. needs to have the original of your W-9 form, so please send the original to this address:
